NEW JERSEY WRESTLING PARTICIPATION "WAIVER" APPLICATION FORM
Purpose of "Waiver" and "Application Form": The purpose of this "waiver" is to allow a wrestler an opportunity to qualify for an event, where, due to some special situations, he/she has not followed the normal qualification procedures. The purpose of the "application form" is to have all the needed information to fairly judge the merits of the waiver while being fair to all of the wrestlers who followed the normal qualifying procedures and did not make the team. "Deposit must accompany application."

State, generally, why a waiver might be considered. Basically we want to know why the NORMAL qualifying procedure (or procedures, if more than one) could not be followed. Then, justify your request by being as specific as possible. Your justification will have to compare to the actual written procedures. (If you need a copy please contact the age level director or state chairman right away) Your dates will have to coincide with the dates of the various qualifying opportunities offered. The more specific and honest your evaluation, the more merit it will have:
Use a separate sheet of paper and attach to this form if you need more room.
