USAW/NJ Advance Purchase of Athlete/Coach Card Contract
This contract is between USA Wrestling/New Jersey

Club Name
1. All current USA Wrestling / New Jersey Chartered Clubs can advance purchase a maximum of 25 competitor cards at one time. The cost of each competitor card will be $25. Chartered Clubs can also purchase a maximum of 5 Coaches Cards at one time. The cost of each coaches card is $25. It is recommended that you purchase only the number of cards you expect to sell in a month.

2 All card roster forms (or other method of registration) must be submitted at least once a month. Roster forms should be submitted whether they have only one name registered or twenty-five. All information on the form must be completed and legible.

3. Unused cards must be returned within 60 days after purchase. Refunds will only be given on cards that are returned by August 1 of the current card year. After August 1 the cards will be considered as purchased.

4. In lieu of refund the club may stipulate to a credit unregistered payments vs the next year prepurchase.

I have read the terms and conditions listed above and understand them. I am aware that any failure to comply with the above terms will terminate my club's ability to pre-buy USAW Cards.
