First, a little on why I made this form. I always note the books they have read in their weekly plan/progress report but the titles get lost there. So I also have the form below. This form is included in the portfolio I keep on the boys. I added extra curricular activities to the bottom half of the form when I realized that those, too, were buried.
Note on books form: We don't always fill out all fields but they are useful for a particularly good or bad book. I have provided 23rows. If you have a younger child, you might read more in a month and will need to add rows. If you have a high school aged child, likely you'll need half or less of these rows. The last column is to help highlight the books used for curriculum.
Note on extra-curricular form: Our homeschool group required 100 hours of community service for high school students wanting to graduate with our group. Therefore, I have a column noting if the activity was community service. Not all scout activities qualify (I decide) for this but luckily our scout troop keeps track of community service and already, both of my sons have earned over 100 hours.
