□ I have submitted my entry and agree to loan my Journal Quilt to Quilts, Inc. for possible inclusion in the special exhibit Journal Quilt Project II: Elements at International Quilt Market and Festival in Houston, Texas, October 25-November 2, 2008, and possibly at other venues selected and arranged by Quilts, Inc.

□ I understand that if my entry is selected for exhibit, I will be notified through e-mail by August 22, 2008.

□ I understand that if my entry is selected for exhibit, I must keep images of my Journal Quilt confidential until the International Quilt Market and Festival in Houston, Texas, October 25-November 2, 2008, is in progress. This includes the webpage or blog of myself or others and not soliciting or participating in publicity about my quilt unless such publicity is arranged by or approved by the International Quilt Festival.

□ I understand that if my entry is selected for exhibit, it is possible that my 2008 Journal Quilt may not be returned until January 31, 2010, depending on venues.

□ I understand and agree that if my entry is selected for exhibit, my Journal Quilt must consist of three layers, finish to 17 x 22 inches, be finished on all four sides, and hang vertically ↕, not horizontally ↔.

□ I understand and agree that if my entry is selected for exhibit, I must submit, by September 26, 2008, my Journal Quilt along with my Artist’s Statement. My Journal Quilt and Artist’s Statement will have a sleeve attached to the top back according to instructions I receive with my acceptance letter in August.

□ I understand that if my entry is selected for exhibit, my Journal Quilt will be insured for a maximum of $100 under your Chubb Group Fine Arts policy, from the time I ship it to you until it is returned to my door.

□ I understand and agree that if my entry is selected for exhibit, I will pay for shipping my Journal Quilt to Festival. I will include with my accepted entry a minimum of $20 (U.S.), either by check drawn on a U.S. bank or by providing a valid MasterCard/Visa number and expiration date. I understand that this fee will cover handling and return shipping costs via U.S. Postal Service. If I prefer another method of shipping, such as FedEx or UPS, which may cost more than USPS, I am responsible for ascertaining that cost and for paying this shipping cost in addition to the $20 minimum. I also understand my check/credit card will be cashed/charged upon receipt.

□ I further agree to permit the image, or detail image(s) of my Journal Quilt, and/or all or part of my statement about my Journal Quilt to be used in articles, ads, promotions, catalogs, books, websites (including any webcast coverage), CDs, current event news coverage, television productions, and/or multi-media productions for and about International Quilt Festival, or for and about the special exhibit Journal Quilt Project II: Elements, or for and about journal quilts and developing creativity through experimentation, without any additional payment or notification to me.

Entries selected from digital images are subject to additional evaluation when the actual quilt arrives in Houston. If the actual quilt is found to differ significantly from the digital image submitted for jurying, the selection committee reserves the right to reject the entry and override the jurors’ initial decision. The decision as to whether the difference is significant is entirely the prerogative of the committee.
