Directions to use forms

The Word format forms may be filled out on the computer using Microsoft Word. All but the Medical and Liability Release forms may be e-mailed to Denise Johnson. You will have to send the check via regular postal mail to her. Bring the Medical and Liability Releases to registration signed by the parent or guardian.

The Acrobat (pdf) forms may be filled out on the computer using Acrobat Reader 6 or newer. They cannot be saved but may be e-mailed from the Acrobat program. All but the Medical and Liability Release forms may be e-mailed to Denise Johnson. You will have to send the check via regular postal mail to her. Bring the Medical and Liability Releases to registration signed by the parent or guardian.
