If you are not a licensed health service provider but you are 21 years old or older you may purchase selected items that are designated with an asterisk (*). Licensed health service providers who wish to purchase the restricted clinical software must first send a copy of your current license via MAIL or e-mail (see above for address). We will notify you of your approval to purchase via e-mail. You will then arrange for purchase from us direct. Your future permission to purchase need only be renewed when your license is renewed.

We will advise you of the status of your order and notify you when it has been shipped.

A new edition to our scale is the Burnout Index. Unlike other Burnout scales that allude to Anxiety and Depression as being components, this scale includes measures of both Anxiety and Depression. You might notice too that Partial Hospitalization is now considered an alternative to RTC and Acute Care placements. Providers and third parties recognize that oftentimes without a PHP level of care, the patient would require more restrictive care.





The Mental Health Quotient is similar in concept to an IQ score but applied to psychological adjustment.

The CLINICAL INDEX OF STRESS is "normed," not "standardized." It is meant to glean as much clinical information as possible, not exclude it. It can be used for patients at any level of care and of any mental OR chronological age: infants, children, adolescents, adults, and geriatrics. Takes about 20 - 30 minutes to administer.

It uses physical symptoms, life events, or endorsable behaviors to infer psychological states.

It allows for entry of a clinical summary and check off of symptoms:

1. May be administered by a clinician as an interview
2. It may be given on a computer to the patient AND/OR informant
3. It may be used to review a clinical record.
4. It allows a clinician to quantify a psychiatric record in terms of severity of illness.

The CLINICAL INDEX OF STRESS automatically provides the following:

 1. A comparative statement of the Total Score for norms (N=1,096) based on level of care
 2. Graphically depicts T-scores and elevations on 8 indexes:

The heuristic aspects of the CLINICAL INDEX OF STRESS are broad (see Biochemical Index example below). It lends itself to basic and applied clinical research. Though it has its own suicidal constellation (early validation has been established; continued validation is ongoing), efforts are underway to develop a homicidal constellation.

We will make a reasonable degree of modifications in the hope of getting you up and running to your satisfaction. We do this at no additional cost to you. The Automated Mental Status Exam provides Multiselect list boxes that allow for single or multiple descriptors of appearance, movements, affect, mood, speech, sensorium, thoughts (clinician estimate of intellect, perception, form, contents), memory, insight and judgment. The Automated GAF provides checkboxes executed by the clinician next to the descriptors of functioning for each level on the DSM-IV-TR™ Axis V--it then calculates a GAF and graphs it by level of care, urban vs. rural settings; it will also plot repeated measures on the Automated GAF form itself and on a separate serial chart (for a single admission or across all admissions). The above is the data entry form. The prescription form output is below:


The Prescription Pad has three variations. The first is like the above. It allows for up to 5 prescriptions at a time. The second has a Tab format with up to 5 possible entries for each of 4 Tabs. This means it will allow up to 20 prescriptions (1 - 20) on the same Prescription sheet. The third is a list for entry of up to 20 prescribed medicines (1 - 20) at one time. It is like the second, but without the Tabs.


Built into the prescription pad are check boxes for No Substitutions, Confirmation buttons for each entry, and check boxes and text boxes for standard prohibitions (e.g., do not operate machinery). On the 1 - 20 Prescription pads, there is a count as to number of prescriptions versus number of confirmations.

Note: The Prescriptions listed above are for illustration purposes only. Some of the dosages, refill amounts, etc. are intentionally nebulous. It is incumbent on the prescriber to get things right. The program cannot do this for you. It will not tell you contraindications, interactions, side effects, dosages, routes of administration, etc.

It is a point of reference for one to further explore through discussions with their Physician and personal research. There is nothing on the Medication Listing that an individual could not obtain by going to a public/university library. This author, Vance Stewart, Ph.D., is a clinical psychologist. A PC with Microsoft® Office 2003 or Office 2007 installed and functional is sufficient.


For the Technically minded: PC (at least a Pentium III, Celeron, or comparable), Microsoft® Windows XP (with Service Pack 2); Microsoft® Windows Office 2007, Microsoft® Access 2003 or Microsoft® Access 97 (you must have Microsoft® Access, a component of Microsoft® Office, to run our software), 12 MB memory to run program, 121 MB of hard disk space, CD ROM drive, VGA or higher resolution monitor, & a pointing device (e.g., mouse).

System Requirements specifically for Microsoft® Windows Vista:



For Microsoft® Windows Vista users, recommended requirements include 1 GHz 32-bit (x86) or 64-bit (x64) processor, 1 GB of system memory, 40 GB hard drive with at least 15 GB of available space, Support for DirectX® 9 graphics with WDDM driver, 128 MB of graphics memory (minimum), Pixel Shader 2.0 in hardware, 32 bits per pixel, DVD-ROM drive, Audio output, and Internet Access. Not all of the recommendatons (e.g., graphics capabilities) are necessary to run our products.

Microsoft® Office 2007 requires a 500 MHz or faster processor, 256+ MB RAM, 2 GB of available hard-disk space, CD-ROM or DVD drive, 1024 x 768 or higher-resolution monitor, Microsoft® Windows XP with Service Pack (SP) 2 or later, or Microsoft® Windows Server 2003 with SP1 or later operating system. Please see specific Microsoft® products for additional requirements/recommendations. We are all on new turf here.
Note: As Microsoft® Windows Vista / Office 2007 are brand new, if there are bugs in Microsoft® Windows Vista / Microsoft® Office 2007, there will be bugs in our products. Microsoft® however, has a reliable history of identifying and providing patches, fixes, and Service Packs based on users' experiences and their own internal processes. These efforts by Microsoft® should remedy any problems you might have. You are also entitled to one free hour of telephone support from FCBHC when you purchase our total package products (in minimum increments of 15 minutes) and one instance of support (up to 15 minutes free) when you purchase selected products.

Note: We have not tested this software under Microsoft® Windows Me/Millinneum Edition. Our experience with Microsoft® Access 2000 has been unsatisfactory. If you try it under other software, e.g., Microsoft® Office 2000 or earlier, it may want to convert the Demonstration or Full Program database. You can try the options it gives you and ignore any error messages.
