Abbreviations:  Cap=Capsules/Caplets  Conc = Concentrate  Inj = Injection/Injectable  Oral Sol = Oral Solution  Tab= Tablets  Span = Spansules  Supp = Suppository; Adjunct therapies are medicines used in combination with other medicines.
Note:  Adrenaline/Noradrenaline are British synonyms for Epinephrine/Norepinephrine respectively.  Edronax (a "noradrenaline reuptake inhibitor" or "NARI" Rx'd outside the USA as an antidepressant) was classified an SNRI in the current scheme.  Strattera (Rx'd in the USA for AD/HD) falls in this same category.
Grayed text followed by two asterisks indicates that the medication is not listed in the 2009 PDR® (#63).  Grayed text only means it may be in a future PDR® supplement, or the pharmaceutical company has changed.
In many cases this is because the medication is no longer made or is off-patent, or is a foreign medication not listed in the PDR®.  In some cases, it may be that the medication was FDA approved subsequent to the publication date of the 2009 PDR® (#63).
A drug brand name followed by a pharmaceutical name(s) in parenthesis indicates prior manufacture or sales of that drug by that entity.
Table by Vance Stewart, Ph.D., a clinical psychologist; clinical psychologists are not medical doctors.    Patients may present with multiple medications of varying age in one bottle; or, may give a history of prescriptions not currently recognized.  It is hoped that this table will aid in such circumstances.
Things change over time; pharmaceutical houses merge, change names, variations on existing medications are offered.  Care was taken in constructing this table.
A variety of sources were used including multiple PDR®'s, the concise edition of Kaplan & Sadock's Synopsis of Psychiatry, and multiple websites; however, it is

Understanding this Current Only Listing chart and the Current & Historical Listing chart
The Medlog is meant to be as simple and straightforward as possible.  However, the evolution of pharmaceuticals is complex.  The following examples should help
clarify the conventions adopted/used to address the changes that have occurred.
 Example 1
In the simplest case, read across for the generic name of a medication, the brand name, and the name of the company
that makes the medication.  Read upwards to determine the type of medication.
Generic (or Chemical name) Brand (Trade name) Company (Manufacturer/Maker)
 Antidepressants
  SSRI (Selective Serotonin Reuptake Inhibitors)
Citalopram Celexa Forest Labs
 Example 2
Both the generic name and brand name are bolded gray with two asterisks, followed by a company name in parentheses.  This indicates that the medication
(generic and brand) are not listed in the 2009 PDR®, #63.   Company may not make the medication anymore or the company name may have changed.
Generic Brand Company
  Antipsychotics
  Phenothiazines
  Acetophenazine** Tindal** (Schering)
 Example 3
The brand name is grayed with two asterisks.  The company name listed in the Company column is in parentheses.   Sandoz used to make the branded product
Mellaril.  Mylan was then making the generic Thioridazine Hydrochloride Tabs at the time of our  review.
Generic Brand Company
  Antipsychotics
  Phenothiazines
Thioridazine Mellaril** (Sandoz)
Thioridazine Thioridazine Hydrochloride Tabs  Mylan
 Example 4
More than one company name is listed.  That means that more than one company make that  particular preparation.  In this case, Bristol-Myers Squibb made
BuSpar; Mylan, PAR, and Watson currently made their respective generics of Buspirone.
Generic Brand Company
  Sedative, Hypnotic, & Anxiolytics
  Anxiolytics
Buspirone BuSpar**  (Bristol-Myers Squibb)
Buspirone BusPRIone HCl Tabs Par
Buspirone Buspirone HCl Tabs Watson
Buspirone Buspirone Hydrochloride Tabs Mylan

  Example 5
Generic name on one line is followed by the same generic name with an ampersand and another generic name on the following line. Olanzapine was manufactured
by Eli Lilly under the brand name of Zyprexa.  They also have a preparation that is a combination of Olanzapine & Fluoxetine, marketed under the label of Symbyax.

Generic Brand Company
  Thienobenzodiazepine**/Combination
  Olanzapine Zyprexa Eli Lilly
Olanzapine & Fluoxetine Symbyax Eli Lilly
The Medlog is always a work in progress.  There are a number of brand name narcotics (Agonist-Antagonist & Combinations) and Adjuncts that may be added to the Medlog.
Examples are:  Actiq (Cephalon), Astramorph/PF inj (AstraZeneca LP), Combunox Tabs (Forest), Duragesic Transdermal System (Janssen Pharmaceutica/Ortho-McNeil),
Kadian Caps (Alpharma Branded Products), Maxidone Tabs C-III (Watson), MSIR Oral Caps, Oral Sol, Tabs (Purdue Frederick), Nubain Inj (Endo Labs), Norco Tabs C-III (Watson),
Numorphan Inj/Supp (Endo Labs), OxyContin Tabs/OxyFast Oral Conc/OxyIR Caps (Purdue Pharma), Synalgos-DC (Wyeth-Ayerst / Women First), Vicoprofen Tabs (Abbott) Opana / Opana ER Tabs--oxymorphone--(Endo Labs) and Zydone (Endo Labs).
Non-narcotic and Anxiolytic combination (Butalbital and Acetaminophen):  Sedapap Tabs (Merz); Adjunct: Clonidine Hydrochloride* Injection (Xanodyne) / Duraclon** Injection (Fujisawa / Elan / Roxane)
When using Edit, Find from your browser's menu bar, you do not have to type in the whole name of a medication.
For example, if you are looking for Anafranil, you could type Anaf and it would highlight it.
Alternately, you can hold down your "F" key and Ctrl key simultaneously to reach the Find function.
