* Apparel Checkout/ Receipt Form
2009 / 2010
All swimmers and divers report to the equipment cage behind the gym to receive the apparel that you will need to wear on Picture Day, which is November 30.  Click here for the  Apparel Checkout/ Receipt Form  that must be signed by the swimmer and parent after receipt of the equipment, and returned to the Coach.
 
* Apparel Order Form
2009 / 2010
Swimmers and divers have already received an  Apparel Order Form  that is due no later than Monday, 11/23 along with payment.  The swimsuit, team T-shirt and swim cap (if needed), are the only items that each member must order.  All other items are optional purchases.
             Seniors, be sure to order your swimsuit from the line that says, "Seniors", so that you get a paw print on your swimsuit!

Athletic Participation Medical Forms

Mandatory Medical Forms
All athletes must submit these two medical forms.

1) 
*Physical Form (dated after May 1 - EACH current school year):  
         
This form combines confirmation of:
Athletic Participation
Parental Consent
Physical Examination

          Make sure that it is complete and signed in all designated areas.  Submission of the completed form (sections 1-5) for physicals performed after May 1, 2009, will qualify an athlete for participation throughout the entire 2009/10 school year (three sport season). 
         Incomplete Physical Forms will not be accepted.  Please note that t
he form requires one student signature, three parent signatures, one physician clearance signature.

2)  *Emergency Care Form (new & updated for EACH season/sport): 
          Must be completed for
each season/sport, i.e. if an athlete participated in fall football, he must complete another emergency care card for any new sport and season.

 

* IMPORTANT:  Be certain to carefully read the  forms list at the top of this pagefor additional forms required for participation in Swim & Dive.
