*Absence Form	2009 / 2010 Complete to the best of your knowledge and turn in at the Nov. 4 meeting.
*Checklist	2009 / 2010 Follow and keep for your records.  Use it to prepare for the Nov. 4 meeting.
*Contract - Rules	2009 / 2010 Sign the last page only and return to the Coach at the Nov. 4 meeting.  Keep and follow what is written on the remainder.
*Database Form	2009 / 2010 For your vital stats.   Complete and  turn in at the Nov. 4 meeting.
Apparel Checkout/ Receipt Form	2009 / 2010 This form needs to be signed by the athlete and parent.  This form will be due after loaned equipment is distributed.
*Comunicable Disease Information Form	2009 / 2010 To be signed by student and parent.  To indicate acceptance of risk.  Complete and  turn in at the Nov. 4 meeting. Go to Athletic Training at the FCPS web site for more.
*Emergency Care Form	2009 / 2010 Mandatory for tryouts and participation in any sport. Complete and  turn in at the Nov. 4 meeting. Click here for important information.

*Field Trip Form	2009 / 2010 Mandatory for ANY activity travel from Westfield.  Complete and  turn in at the Nov. 4 meeting.

Letter Jacket Order Form	2009 / 2010 Follow instructions on the form.  Bring your loose “W”, a check made out to WABC and your order form to the Activities Office. Contact one of the Athletic Booster board members if there are any questions. .
NCAA Eligible Course List . Use the school code 470406 (copy, then paste code in box at the NCAA site linked here)	2009 / 2010 CORE  COURSES  TO  BE  TAKEN  BEFORE GRADUATION  FROM  WESTFIELD. To become certified for athletic participation at the college level, certain criteria must be met. This is a list of specific NCAA eligible courses for Westfield High School. Also, see the NCAA & NIAAA links below.

Athletic Participation Medical Forms

Mandatory Medical Forms All athletes must submit these two medical forms. 1)  *Physical Form (dated after May 1 - EACH current school year):             This form combines confirmation of: Athletic Participation Parental Consent Physical Examination           Make sure that it is complete and signed in all designated areas.  Submission of the completed form (sections 1-5) for physicals performed after May 1, 2009, will qualify an athlete for participation throughout the entire 2009/10 school year (three sport season).           Incomplete Physical Forms will not be accepted.  Please note that the form requires one student signature, three parent signatures, one physician clearance signature. 2)  *Emergency Care Form (new & updated for EACH season/sport):            Must be completed for each season/sport, i.e. if an athlete participated in fall football, he must complete another emergency care card for any new sport and season.

* IMPORTANT:  Be certain to carefully read the  forms list at the top of this page,  for additional forms required for participation in Swim & Dive.
