INSTRUCTIONS
If you have siblings who are CF associates, use your parents as HUSBAND and WIFE. If you have children who are CF associatesi, use a separate form and put your name as HUSBAND or WIFE, as appropriate. If your grandparents and/or aunts and uncles were CF associates, use a separate form with the grandparents as HUSBAND and WIFE, as appropriate. Use a separate form for each family, and use an extra form if there are more than six children. If you have no other relatives who are CF associates, you may submit a form for you and your children. If you have other relatives who are CF associates and there is no one else to account for them, please fill out a separate form for them, so that they'll be represented. HELP US ACCOUNT FOR ALL YOUR FAMILY MEMBERS WHO ARE CF ASSOCIATES!.
