The Neighborhood Care Team (NCT)
NCT is a short-term mental-health outreach case-management program for adults in Upper Manhattan. It is staffed by bilingual, and bicultural personnel who are sensitive to the needs of individuals and families with histories of mental illness. It links clients and their families with the support services necessary for re-integration and stability within the community. The program prevents decompensation and repeat hospitalizations, and reduces the risk of family disintegration. It handles a caseload of 50 high-risk individuals and families who, besides coping with mental illness, are often recent immigrants acclimating to a new environment. Staff members make case management home visits. They also assist clients and their family members to better understand how mental illness impacts the client, his or her family, his or her social network and his or her vocational functioning. They also teach them to recognize the early symptoms of recurrent mental dysfunction.
