Have I placed God before all my likes, selfishness, passions, laziness, pride, vanity, and human respect, or have I made these the supreme values of my life?
Have I consented to serious doubts about my faith or placed myself in danger of losing it?
Have I despaired at times not trusting in God and in my salvation? Have I cultivated a pessimistic spirit and low self-esteem, not believing in myself and the tremendous love that God has for me?
Have I abused my confidence in God being overconfident in His forgiveness in order to sin more freely?
Have I murmured against God for the things I do not like?
Have I used the name of God thoughtlessly, in cursing or in blasphemy?
Have I used the name of the Lord Jesus in vain, as a curse word, an expression, or in a frivolous way?
Have I used foul and irreverent language?
Have I been absent out of my own fault from Sunday Mass or on the Holy Days of Obligation?
Have I received Holy Communion is the state of mortal sin?
Have I disobeyed my parents or other lawful superiors, while under their authority?
Have I treated my parents or superiors with disrespect or lack of appreciation?
Have I hated others, held grudges, or been unfriendly to them?

Bless me, Father, for I have sinned. It has been________since my last confession. These are my sins:
ACT OF CONTRITION
O my God, I am heartily sorry for having offended Thee, and I detest all my sins, because of Thy just punishments, but most of all because they offend Thee, my God, who art all-good and deserving of all my love.
I firmly resolve, with the help of Thy grace, to sin no more and to avoid the near occasions of sin.
Have I let myself be controlled by jealously and envy?
Have I despised others? Believed myself to be superior to them? Have I ridiculed others, criticized them or made fun of them?
Have I treated others poorly in word or action?
Have I abused myself by excessive use of alcohol, drugs, or gluttony?
Have I entertained impure thoughts, memories, imaginations, or desires? Have I viewed immodest pictures, movies, or TV. programs? Have I read suggestive magazines or books? Have I committed impure actions (alone or with others)? Have I committed impure actions with others by acts of adultery, fornication (pre-marital sex) or homosexual acts? Have I been modest in dress and kept control over my senses? Have I avoided the occasions of sin?
Have I used contraceptives?
Have I in any way participated in an abortion?
Have I taken anything that was not my own? Have I been guilty of vandalism? Have I cheated anyone out of something that is justly theirs?
Have I desired other's possessions, not being satisfied with what God has given me?
Have I been guilty of a deliberate lie for any reason?
Have I spoken falsely or poorly of others, spoken behind their backs, or cooperated in gossip about them.
Have I judged others and made these judgments known to third parties?
