PDF FILE REMOVAL NOTIFICATION
This is to notify you that the PDF version of "John's Medical History Binder" has been removed for safety purposes. Since it has the Durable Power of Attorney For Healthcare and my signature, it has been removed from wide distibution in PDF format. The PDF file is now going to be kept on the computers of all care providers, as well as on USB MicroDrives for hospital admisssions. Plus hard copies as needed.
