Affidavit

I, ______________________________, do hereby certify that I elect to use my alternate final exam score in lieu of actually taking the final exam in Chem 637.  I understand that my present class ranking and projected grade may change as a result of the performance of those class participants who elect to take the Chem637 final exam. This could potentially result in either a raising or a lowering of my rank in the class and thereby change the final grade that I earn based on my rank in class.
