Clinical Questionnaires

 

For certain clinical situations, questionnaires are used to provide information of a person’s medical-neurological-psychiatric history. Questionnaires and charts help to organize, provide valuable information on past treatment efforts, and document the characteristic time course of certain symptoms.

 

I suggest we (you as a prospective patient, and I) discuss the need for one or more of these questionnaires on an individual basis prior to your initial consultation. If we decide a questionnaire may help in your specific situation, it would be most useful to bring it filled out to the initial consultation.
